Below is a sample letter that contains the basics needed to dispute inaccuracies in your credit report. This can be used for all 3 major credit bureaus: Experian, Equifax and Transunion as well as any other credit reporting agency.

I request that you investigate this account and, if it cannot be verified, remove it from my credit file.

If the entry is removed, I request that you make notification according to 611(d) of the Fair Credit Reporting Act.

Give maiden name if applicable

Give addresses over past five years, including zip code

Give spouse’s full name and social security number if applicable

Copy of driver’s license or other photo ID.

Date of Birth

Include copy of your credit report with disputed item highlighted
